Requirements:
Opt-in Form & SMS Consent must be visible: The opt-in form needs to be visible and not hidden behind a multi-step form (like a Typeform or other multi-step form where the phone field and SMS consent is on the second page).
Only One Use-Case Per Checkbox: Each checkbox and consent can ONLY contain 1 use case. If you want multiple use cases (for example transactional and marketing), you must use multiple separate checkboxes, each with the consent text.
Instructions for "Mixed" use cases
If you're using a "mixed" use case with multiple use cases, you'll need multiple consent checkboxes like this:

SMS consent must be by itself as a channel: The SMS consent cannot contain multiple channels like sms, email, phone etc. SMS consent must be by itself.
SMS Consent Must Be Optional: Cannot be checked or required by default.
Privacy Policy & TOS: The Privacy Policy and TOS should be hyperlinked. If not available, you can display the full URL.
Phone Number Field: Phone Number must be optional
Landing Page Navigation: The landing page must link to the following pages specifically: About, Contact, & Products / Services. It will be rejected if it does not contain links to these pages (or the content) in the navigation.
